GETTING STARTED.

STEP 1  

Request an appointment by completing the form to the right.

STEP 2

Our team will complete insurance verification and request a referral from the physician.

STEP 3

A member of our team will contact you to complete intake forms and schedule an evaluation.

STEP 4

The assigned speech pathologist will complete an evaluation report and then send the completed report to the physician for signature.

STEP 5

The signed evaluation report is sent to insurance company for authorization of services.

STEP 6

You will receive a call to schedule speech therapy appointments once authorization of services has been completed. A speech pathologist will discuss a treatment plan.
